「メイク・ニュー・スタンダード次世代研究事業」の申請資格審査申請書（様式 A）提出フォームです。下記にて必要事項を入力して下さい。※博士後期課程（医学博士課程）期間に休学をしている申請書者のみ

This form is for submission of the evaluation document (formA) for "the Make New Standards Program for the Next Generation Researchers".※ Applicants who took a leave of absence during their doctoral programme (MD programme) only
